ADOPTION AGREEMENT

TERMS OF ADOPTION

1. You agree to adopt a male/female dog, age __________, breed__________ color ________, with the call name of “______________.”

You agree to an adoption fee of ______________

Rescue will not place a dog known to have an unsound temperament. However, no one can predict how any dog will react in a given situation, especially without complete knowledge of its life experiences. Therefore, the Adopter is strongly urged to exercise prudence and caution in introducing and adoptive into a new situation until the dog had become fully adjusted to its new environment and the Adopter has had an opportunity to become familiar with the dog’s unique personality.

2. No dog will be placed in a new home which does not provide a fenced yard. A dog must never be allowed to run loose. It should always carry identifying tags, be tattooed with an identifying name and number, and have a microchip implant. The dog will require at least 1-2 hours of exercise daily. They should also be in a home where they will be sleeping in the home and not left outdoors at night.

3. The Adopter must be prepared to demonstrate their ability and willingness to provide proper high quality food, water, medical care, and a loving, safe, healthy, and clean environment for the adoptive dog. The adopter promises that this dog’s socialization will continue. Medical care includes: a) an annual check up and follow up on any occurring health problems b) a heartworm diagnostic and prevention program must be maintained c) internal and external parasites must be checked regularly and eliminated by veterinarian approved methods. External parasites to include all types of fleas, ticks, and mites. Internal parasites to include hookworm, roundworm, whipworm, and heartworm. Even if the dog has already had a basic obedience course, we urge a “refresher” course to establish respect and a close rapport more rapidly between dog and owner. Always be sure that the trainer is familiar with the dog’s temperament.

4. The ownership and responsibility for the adopted dog belongs solely to the Adopter. However, if for some valid reason, the adoption does not work to the advantage of the new owner and the dog, MGPR SERVICE must be contacted and given the option of reacquiring the dog before any further placement. The adoption fee will not be refunded. The MGPR SERVICE has the right to cancel this adoption agreement at any time if the adoptive dog has any outward signs or substantiated reports of physical and/or mental abuse.

5. In as much as is practical, a MGPR SERVICE member to evaluate its qualifications as an adoptive home will visit each prospective adoptive home.

A follow-up visit and supportive advice by a participating MGPR SERVICE member will be strongly urged following placement of the dog through Rescue.

The MGPR SERVICE makes no representations or warranties regarding the temperament or physical condition of adoptive dog. All warranties including implied warranties of merchantability and fitness for a particular purpose, where they are applicable, are hereby specifically disclaimed.

6. The MGPR SERVICE shall not be responsible for any bodily injuries or property damage caused by any adopted dog or by the actions of its Adopter. The Adopter hereby specifically assumes sole responsibility for, and agrees to hold the MGPR SERVICE harmless from, any and all loss and expenses (including legal fees) by reason of liability imposed by law upon MGPR SERVICE, or any of it’s officers and members because of bodily injuries or death to any person or persons including the Adopter, or any damage to property arising out of or inconsequence of the placement of the dog, howsoever such injuries, death or damage to property may be caused, whether or not the same may have been cause, or may be alleged to have been accused, by negligence of the aforementioned parties or any of their employees, agents, or members, or any other person.

7. Should it be necessary for the MGPR SERVICE to take legal action to recover the dog, or otherwise enforce the provisions of this agreement, the undersigned Adopter agrees to pay all expenses incurred by the MGPR SERVICE, including court and legal costs.

8. The Adopter owners must advise the MGPR SERVICE, in writing, of a change in address and/or phone number so that we may update our records.
